The role is funded by Royal Cornwall NHS Charities (Capt Tom’s fund) & was a joint BID supported by all the Hubs within Penwith to encourage those motivated to connect with their local community, partake in social activities or gain support in various areas of health, wellbeing, social inclusion etc. Each Hub offers a multitude of services & support, all individual but all with people & the community at their heart.
I work closely with Social Prescribers, Mental Health Teams, GP & other professionals, foster links within the Community, not just with the Hubs but also building relationships with Community Groups such as the U3A, Carer Support, Cuppa Companions, Memory Cafés, Pain Cafes, & 50 plus to encourage shared knowledge & Partnership working in order to reach a variety of people who might benefit from the Community Buddy support.
Whilst there is a referral form should it be required, many I’m sure, given the relationships being forged, will also call or email me to discuss & refer.
The intention is that those interested in & motivated to attend a Hub but face barriers in doing so, can be supported via the Community Buddy, whether that’s meeting the client at or taking them to the relevant Hub. The ultimate goal will always be for the client to attend independently & utilise the support & social interaction available via their chosen Community Hub(s). My role is only part-time (22.5 hours per week) & so in order to help as many clients as possible, it’s important to encourage independent access after a period of one-to-one support.
It’s my understanding that Social Prescribers identified a ‘missing link’ in the ongoing support of patients/potential service users following their vital input, in that whilst they recommend relevant Hubs, many needed additional one-to-one support to actually attend & without it would be unable to utilise services even when motivated to do so. The Community Buddy role aims to fill that gap & meet the need for a helping hand & a little extra support & encouragement. It’s a really exciting role, one I’m sure will be well received & will make a positive difference within the local community & I’m so grateful to be part of the journey….
